Thank you for for all the help! I learned a lot through this troubleshooting process, Exchange is up to date, and we have some best practices implemented. I have no idea why this account was there and why it was set up in this way. This is why documentation is so important. I deleted the "cgatepvs" user and BOOM, everything works! Its been decommissioned for about 2 years now. The previous consulting group had Websense installed on the Exchange server. I thought maybe "Content Gateway?" so I googled it, and I got hits for " I also don't use the default "users" OU :-) This was nothing I created because I put descriptions in service accounts.

So I am sitting here thinking what the heck the "cgatepvs" could be. In such a case, you can switch to a stable network connection to solve the latter.

When I click next, it prompts me for credentials (which is odd to me, because it should just pass my windows credentials) Any credentials I try and type in are rejected (I have tried domain\username, and my email address). When I open Outlook 2016 for the first time, it populates my name and email as I would expect. I tried the registry entries as described. Basically the issue appears exactly like described in this KB article I am unable to create a new email profile in Outlook 2016 on our LAN. I have not run across a problem this weird in a very long time.
